Gestational Diabetes and Blood Sugar Regulation
Gestational diabetes develops when hormonal changes during pregnancy interfere with the body’s ability to use insulin effectively. Insulin is the hormone that helps move glucose from your bloodstream into your cells. When this process is disrupted, blood sugar levels can become persistently elevated.
With gestational diabetes, there are two primary times to monitor these levels in pregnancy:
Fasting Blood Sugars: These are measured after a period of not eating—typically overnight. Fasting blood sugars provide insight into how the body is managing its baseline glucose levels, and elevated fasting levels may indicate that the liver is releasing extra glucose or that your insulin response isn’t strong enough, even when the body is at rest.
Postprandial Blood Sugars: These levels are checked after meals when the body is processing a surge of nutrients. Generally, daytime activities and the natural insulin response help bring these levels down more quickly compared to fasting levels.
Why Elevated Fasting Blood Sugars May Pose Greater Risks
New evidence suggests elevated fasting blood sugars may increase more perinatal risks compared to post-meal spikes.
Overnight, when you're sleeping and less active, your body doesn’t have the benefit of movement to help lower blood sugar levels.
If your fasting blood sugar is elevated, it means that you—and by extension, your baby—are exposed to high levels of glucose for many consecutive hours. This period of sustained high glucose levels can lead to a higher nutrient (or glucose) supply to your baby. In response, baby produces more insulin to store it away.
While insulin is crucial for growth, excessive amounts can lead to conditions such as macrosomia (a larger-than-average baby) and may even increase the risk of neonatal hypoglycemia once the baby is born. Plus, the prolonged exposure to elevated blood sugars can stress the developing fetus, potentially affecting long-term metabolic health.
Compared to fasting blood sugars, post-meal spikes (postprandial elevations) occur right after eating; while these spikes temporarily raise your blood sugar, your daytime activity and insulin response typically help bring them down more quickly.
This means that, unlike post-meal increases which are often short-lived, elevated fasting levels may represent a prolonged exposure to high glucose, which can be more challenging for your body—and your baby—to manage.
So What Exactly is Happening Overnight?
During sleep, your body is in a state of repair and renewal. Normally, lower activity levels mean that your metabolism has a chance to “rest.” However, when fasting blood sugars remain elevated the liver may continue to produce and release glucose even when it isn’t needed, compounding the problem of high blood sugar.
The prolonged hyperglycemic environment can make your cells even less responsive to insulin, setting up a cycle of further increases in blood sugar.
Constant exposure to high glucose levels may also affect the placenta, potentially impairing its ability to regulate nutrient exchange and support fetal development.
Tips to Help Manage Elevated Fasting Blood Sugars
If you’re noticing a trend of higher-than-target fasting blood sugars, always speak with your healthcare provider to devise an individual, best strategy for you and your baby.

Aim for a Balanced Snacks:
Consider a small, balanced snack in the evening that is low in simple carbohydrates. For example, foods high in fiber and protein can help maintain steady glucose levels throughout the night.
Adjust Meal Timing:
Eating dinner at a different time can help ensure that your blood sugar levels have a chance to stabilize before bedtime.
Optimize Insulin Regimens:
If you are on insulin or other medications, the timing and dosage—especially of long-acting formulations—might need adjustment to better control fasting levels.
Ask for More Frequent Reviews:
More frequent consultations with your healthcare team may help fine-tune your regimen based on your blood sugar patterns. Never be afraid to reach out to them sooner if needed!

Consider keeping a daily record of your blood sugar readings. Tracking patterns over time can provide insights into how your body responds to different foods, medications, and activity levels.

A post-dinner walk or light stretching can help facilitate glucose utilization before bedtime. Try to avoid vigorous exercise right before sleep, as this can sometimes interfere with your rest (which can also affect fasting blood sugars).
Get Quality Sleep:
Stress and poor sleep can influence blood sugar levels. Incorporate relaxation techniques, maintain a regular sleep schedule, and create a comfortable sleep environment to support overall metabolic health.
